If you have lost, or have already turned 16 and want to change your "Junior" card for an "Adult" card or want a new ACUC card, please follow the instructions below:

If you took your ACUC course in any country in the world:

– The fastest way is by filling out and sending this online PDF questionnaire.

The cost of issuing a new lost card is as indicated in the application questionnaire, depending on the selected modality.

If you wish a new card because of a name change, we will need copy of some type of official document where both, the previous and the actual names show.

If you request a physical card, instead of a digital one, and if you need it urgently, we will send you a confirmation letter by email, which will include your ACUC diver number, certification date and type of qualification, at no additional cost.

ACUC accepts Visa or MasterCard only. ACUC will not send the new card until it has been previously paid. ACUC does not make COD shipments.

Please allow between 2 and 4 business days for the issuance of the new physical card (in the case of physical cards, ACUC is not responsible for delivery delays caused by third parties).
